NEW PATIENT INFORMATION
Please fill out and bring the New Patient Paperwork with you to speed up your visit!
We strive to remove any anxiety a new patient may have by letting them know what to expect up front.
BEFORE YOUR VISIT:
The new patient experience will begin with contacting our office to schedule a new patient appointment. Then the patient will either download the necessary paperwork from this website, or we can email it directly to them. By completing the documents prior to the visit, this saves time in the office for the patient, helping us to get started with the consultation and examination right away. We get all of the information needed to help the patient with any health challenges they may be experiencing, and gain an idea of what their health and wellness goals may be.
WHEN YOU ARRIVE:
Upon arrival in the office, the patient is
greeted at the front desk, and asked to provide their paperwork if needed. The patient is then led into our consultation room, where their vitals will be taken. Before the doctor starts the appointment, the patient is shown a video briefly explaining chiropractic for those who have never experienced the benefits in the past.
DURING YOUR VISIT:
Next, a comprehensive consultation is done with the doctor to go over the patient’s goals and challenges, and answer any questions both the doctor and patient may have. After that, the doctor performs an in depth examination, including ranges of motion, orthopedic/neurologic testing while in the consultation room. The appointment then moves to the Scan Room, where computerized EMG and Thermography nervous system scans are performed to determine areas of nerve interference, and where x-rays are taken if necessary. This initial visit usually lasts approximately 60 minutes.
YOUR NEXT VISIT:
The doctor will prepare her findings from your initial appointment before your follow-up appointment, this appointment is referred to as the Report of Findings. During the follow-up visit, the doctor sits down with the new patient and goes over all examination findings, answers any questions the patient may have, and puts together a plan of care to get the best possible results. This care plan will help the patient meet their goals and expectations. After the report of findings appointment is over, the patient is given a brief tour of the office, and then their care begins with their first adjustment.
